- Current Eyeglasses
- Current Sunglasses
- Current Contact Lens Prescription (if available)
- List of Medications currently being taken
- Vision and Medical Insurance card (many times your medical insurance will cover what vision insurance does not)
- Completed New Patient Forms (see link above to download forms)
